Before any athlete can participate in Conditioning and trying out for any sport, there are 2 things that need to be completed:
#1 – If you are a returning athlete from 2019-20, and your physical is on file in the HSE Athletic Office, that physical is good for the 2020-21 School Year. If you did not turn a physical into the Athletic Office and want to try out for a sport you will need to use the Physical Form below and turn it in when we re-open. If you are an incoming Freshman, and had a physical on file at your junior high for the 2019-20 school year, that physical will be brought over to HSE and can be used for your Freshman year!!
#2 AFTER MAY 1ST– Rank One Online forms must be completed by parent and athlete together. You can find the online forms under the MORE Tab. Click on Online Forms- Fill form out completely.

The IHSAA has changed their entire physical form for the 2020-21 school year. Only the new form will be accepted. On the physical, NO STAMPED SIGNATURES WILL BE ACCEPTED!
- The physician, nurse practitioner or physicians assistant signature must be hand-written. NO signature stamps will be accepted!
- The physician, nurse practitioner or physicians assistant signature AND licenses number must be on page 2
- If we receive a IHSAA physical with a stamped signature, it will not be accepted and considered invalid.
- These changes come directly from the IHSAA and are not rules from HSE Athletics.
Parents and students must sign at the bottom of page 1 and on page 5. That process has not changed.
