IHSAA Physical and Rank One Online forms must be completed before you can begin conditioning

Before any athlete can participate in Conditioning for any sport, there are 2 things that need to be completed:

#1 – IHSAA Physical must be complete and turned into the HSE Athletic Office (must be completed after April 1, 2017 to be valid for the 2017-18 school year.

#2  Rank One Online forms must be completed by parent and athlete together.  You can find the online forms on the tool bar across the top of the website. Click on Online Forms- High School, then click on Online Signature Forms. Fill form out completely. INCOMING FRESHMAN can complete their forms NOW!!

The IHSAA has changed the doctor’s signature process for physicians performing IHSAA physicals.  NO STAMPED SIGNATURES WILL BE ACCEPTED!

  • The physician, nurse practitioner or physicians assistant signature must be hand-written.  NO signature stamps will be accepted!
  • The physician, nurse practitioner or physicians assistant signature AND licenses number must be on page 2
  • If we receive a IHSAA physical with a stamped signature, it will not be accepted and considered invalid.
  • These changes come directly from the IHSAA and are not rules from HSE Athletics.

Parents and students must sign at the bottom of page 1 and on page 4.  That process has not changed.
